Propiamcaine is an amide anesthetic, similar in intensity and speed to lidocaine, but with a longer duration and a weaker effect on vasodilation. Toxicity is lower than ridoca. Clinical use for local anesthesia, especially for patients who do not use adrenaline. 【 Pharmacology 】 The local anesthetic effect of 3% solution is similar to that of 2% lidocaine plus epinephrine. The onset effect is relatively slow, about 6 ~ 7min, the maintenance time is about 1.5 ~ 2h, and the mucosal penetration is strong. Epinephrine lengthens the effect slightly. The PPB is 55% and T1/2 is about 1.5h. In liver metabolism, its metabolite nitroso-toluidine oxidizes hemoglobin to form methemoglobin. It can be transported to the fetus through the placenta. [Adverse reactions] methemoglobinemia, cyanosis, tachycardia, headache, dizziness and weakness can be produced when the dosage exceeds 600mg at a time. [Precautions] Patients with anemia, congenital or acquired methemoglobinemia, heart failure, respiratory failure or hypoxia should not be allowed. Do not use for obstetric anesthesia. [Usage and dosage] Infiltration anesthesia: use 0.5% ~ 1% solution, the duration of action is 1 ~ 1.5 hours. Nerve block anesthesia: 1% ~ 2% solution, the duration of action is 2 ~ 3 hours. Epidural anesthesia: use 1% ~ 1.5% solution 10 ~ 30ml, the duration of action 2.5 ~ 3.5 hours. A maximum dose of 600mg.
